Fibromyalgia is a chronic condition characterised by widespread musculoskeletal pain, fatigue, sleep disturbance, and cognitive difficulties often referred to as "fibro fog." It affects an estimated 2–4% of the UK population, predominantly women, and can be profoundly debilitating. While there is no cure for fibromyalgia, massage therapy has a growing evidence base as an effective component of a multi-modal management strategy.
Fibromyalgia is now understood as a disorder of central sensitisation — the nervous system becomes hypersensitive to pain signals, amplifying normal sensory input into pain. This means that the pain experienced by people with fibromyalgia is real and physiological, not imagined or psychological. The condition is often accompanied by tender points — specific areas of the body that are particularly sensitive to pressure.
A systematic review published in the journal Pain Medicine found that massage therapy produced significant improvements in pain, anxiety, and depression in fibromyalgia patients. Other studies have demonstrated improvements in sleep quality, fatigue, and overall quality of life.
The key finding across the research is that moderate-pressure massage is more effective than light-touch massage for fibromyalgia. This is thought to be because moderate pressure stimulates the vagus nerve and activates the parasympathetic nervous system, producing the neurological changes that reduce central sensitisation.

The pressure must be carefully calibrated — firm enough to stimulate the parasympathetic response, but not so firm that it triggers a pain flare. Communication between therapist and client is essential, and the treatment should be adapted based on how the client is feeling on each visit.
